Since we offer two types of donations, you'll need to read the first page of the form to determine which type best fits your needs.
If you decide to request a corporate-level donation, you must complete and submit the application at least five weeks before your event. Requests that do not meet this requirement will not be considered.
We will respond to all donation requests within four weeks.

Smiley and his Cookie Cruiser visit community events throughout the year. If you would like to invite Smiley and/or his Cookie Cruiser to your special event, click the link below to fill out our online form. This form must be submitted at least six weeks before your event.
Unfortunately, we cannot finalize Smiley's schedule until four weeks prior to an event, so we'll respond to Smiley Mascot and Cookie Cruiser appearance requests at that time. Please note that Smiley does not make appearances at private events.
